Class/Challenge Information

You must register and PAY FOR your challenge at least 5 days in advance. No registraion will be held without payment. NO payment will be accepted at the class. 

All participants must be 15 years old to participate.
Please bring a state or military identification card or birth certificate to the first class for age verification.

All participants must pass the prerequisite skills test first.
This includes:

• 300-yard swim with 100 yards of front crawl, 100 yards of breast stroke and 100 yards of either front crawl or breast stroke with rhythmic breathing.
• Swim 20 yards, surface dive to retrieve a 10-pound object from a depth of 7-10 feet, and swim back 20 yards.

If you are UNDER 18 you must fill out this WAIVER, have it signed by your parent or guardian and bring it to class.

Important Class Information:

• You must attend all of the session and participate in all activities to successfully complete the course. There will be NO MAKE-UP CLASSES SCHEDULED!
• All required course materials are included in the cost of the class.
• You will not be registered for the class until we have received payment.
• If you prefer to register via mail, please send payment to:

• A minimum of six (6) participants is required. You will be notified if any program is canceled due to insufficient registration.
• If you must withdraw from a class, please write/fax a letter at least five (5) business days before the class begins. You may choose from the following options: a) transfer to another session (a fee of $25 will be applied) or b) receive your money back less a $25 processing fee.
• If you do not cancel five days before your course you will not recieve a refund.
  
• If you do not pass the pre-course test, you may re-enroll in a later class session or you may request a refund for amount you paid minus $75 (in processing and supply fees).
• Refunds take 2 – 3 weeks.
